Market Overview
Private Medical Insurance in Vietnam
Vietnam is one of the fastest-growing insurance markets in Southeast Asia, driven by rapid economic development, a large and young workforce, and rising demand for quality healthcare. The country has a growing expat population concentrated in Ho Chi Minh City and Hanoi, and a substantial FDI sector making it one of the most important markets for international group medical insurance in the region.
Vietnam's regulatory framework for insurance, governed by the Insurance Supervisory Authority (ISA) under the Ministry of Finance, creates a distinctive dual market. Fully domestic Vietnamese companies must use onshore-licensed insurers, while foreign-invested enterprises (FDI companies) are permitted to access offshore insurance providers under Decree 46/2023, subject to strict criteria around insurer financial strength.
As of 1 July 2025, VAT on offshore insurance premiums has been abolished, significantly reducing the cost of compliant offshore plans for qualifying FDI entities. The onshore market includes major domestic insurers such as Bao Viet and PTI alongside international names like AIA, AXA, Allianz and Liberty.
Permitted with Conditions
Entities with foreign investment (FDI companies). Vietnamese citizens working for qualifying employers. Not available to fully domestic entities.

For Employers
What Every CEO & HRD Should Know in Vietnam
Statutory Obligations: VSI
All employers in Vietnam must contribute to Vietnam Social Insurance (VSI), which includes compulsory health insurance under the Vietnam Health Insurance Law. Contributions are calculated on monthly salary, with both employer and employee portions set by regulation.
VSI health cover provides access to public hospital treatment only. It does not cover private international hospitals, specialist clinics, dental, or medical evacuation. Supplementary IPMI is essential for all international employees.
Tax Treatment, Post July 2025
VAT abolished: As of 1 July 2025, VAT on offshore insurance premiums for qualifying FDI entities has been eliminated. This meaningfully reduces the cost of Decree 46-compliant offshore IPMI plans.
CIT deductibility: Employer-paid premiums for compliant plans may be deductible under corporate income tax rules, within statutory limits. PIT treatment of employer-paid premiums as a benefit-in-kind continues, seek current tax advice before structuring.
Decree 46, What Qualifies Offshore
Under Decree 46/2023/ND-CP, offshore insurance is only available to FDI entities. The offshore insurer must have minimum USD 2 billion in equity capital and at least 10 years of continuous operation. Most major global IPMI providers meet this bar, always confirm before placing.
Fully domestic Vietnamese companies must use ISA-licensed onshore insurers. Working with an internationally based adviser like Vennsure ensures your offshore plan structure is compliant with Decree 46 requirements.
Key Risk: Medical Inflation
Vietnam's private hospital sector is experiencing significant medical cost inflation, particularly at JCI-accredited facilities in HCMC and Hanoi. Premium renewal increases of 15–25% are not uncommon for groups with poor claims experience.
Structuring your group plan with an appropriate deductible, co-pay, or outpatient cap can significantly reduce premium inflation at renewal. Contact Vennsure for a plan design review before your next renewal cycle.
